(Including supported independent living (SIL) assessments) – A functional capacity assessment is designed to assess a participant’s functional performance and capacity in everyday activities in their home and in the community. The assessment is typically conducted within the participants home involving interview, observations, and formal assessments. The assessment report will analyse the participant’s functional capacity and identify areas for support. Support and therapy recommendations to improve participant function, independence and quality of life will be provided throughout the written report.

We are qualified to complete NDIS CHM applications. CHMs typically involve structural changes (e.g., removal of bathtub to install an accessible shower) or may involve multiple areas of the home (e.g., bathroom and access). The Occupational Therapist will conduct a home assessment with the participant to identify environmental barriers and recommendations to improve accessibility, independence and safety. A joint assessment with a qualified builder/advisor may be required following the initial home assessment to provide an accurate quote and ensure feasibility of works. A written application to NDIS is completed and submitted with the outlined scope of works, recommendations and quotations.

We can provide assessment and recommendations for minor home modifications such as the installation of grab rails to improve a participant independence, transfers, mobility and safety.

We can conduct assessments and prescribe AT to determine suitable solutions to help participant's achieve their goals, improve their independence and safety. AT assessments will involve physical trials to ensure suitability, safety and satisfaction of use, followed by the appropriate application process for funding. 

We provide therapeutic intervention to participants including skill development and rehabilitation services to assist participants to achieve their goals and improve their independence.

We can conduct falls risk assessment (including home assessments to identify falls risks or hazards) and implement preventative measures to improve safety.
